چکیده

Dupilumab blocks the shared receptor component of IL-4/IL-13, key and central drivers type 2 inflammation. In patients with asthma or CRSwNP, a transient increase in eosinophils was observed dupilumab treatment which declined over time rarely clinical consequence. The effect on blood assessed across indications. Median (95% CI) percent change from baseline measured dupilumab-treated uncontrolled moderate-to-severe (QUEST [NCT02414854]: N=1,902; TRAVERSE [NCT02134028] dupilumab/dupilumab-arm: N=1,013), severe CRSwNP (SINUS-52 [NCT02898454]: N=303; EoE [NCT02379052]: N=47), AD (CHRONOS [NCT02260986]: N=425) patients. asthma, increased by 9.2% ([4.3–14.3]; P=0.001) at Week 4, returned to 24, fell below 52 (−12.3% [−15.9 −7.7]; P=0.03). This trend sustained 96 for treated who continued open-label TRAVERSE. 16.2% ([–5.9 34.0]; P=0.26) rise seen 16 resolved 24. No period AD. Hypereosinophilia (>3,000cells/μL) rare, few discontinued due eosinophilia (QUEST: 7 discontinued/52 cases, TRAVERSE: 2/3, SINUS-52: 1/2), majority did not require corrective treatment. Rare cases EGPA eosinophilic pneumonia were reported; causal association has been established. Transient early elevation generally levels time. Eosinophils

Expression of cysteinyl leukotriene receptor GPR17 in eosinophilic and non-eosinophilic chronic rhinosinusitis with nasal polyps.

BACKGROUND The cysteinyl leukotrienes (cysLTs) are proinflammatory lipid mediators that act on the type 1 cysLT receptor (CysLT1R) in the pathogenesis of chronic rhinosinusitis with nasal polyps (CRSwNP). GPR17, a G protein-coupled orphan receptor with homology to the cysLT receptors, has been proposed as a damage sensor during inflammation. However, the expression and correlation of GPR17 and ...

Role of interleukin-17A in the eosinophil accumulation and mucosal remodeling in chronic rhinosinusitis with nasal polyps associated with asthma.

BACKGROUND Interleukin (IL)-17A is a highly inflammatory cytokine with a robust effect on stromal cells in many tissues. Although IL-17A is known to be associated with inflammatory lung disorders by triggering an accumulation of neutrophils, the effect of IL-17A on the upper airway is still uncertain. The expression of IL-17A and its role were investigated in the nasal polyps of chronic rhinosi...

Role of Interleukin-10 on Nasal Polypogenesis in Patients with Chronic Rhinosinusitis with Nasal Polyps

BACKGROUND AND OBJECTIVES Interleukin 10 (IL-10) is a potent anti-inflammatory cytokine. The dysregulation of IL-10 is associated with an enhanced immunopathologic response to infection, as well as with an increased risk for developing numerous autoimmune diseases. In this study, we investigated IL-10 expression in chronic rhinosinusitis with nasal polyps (CRSwNP) and assessed the possible role...

Genetics of eosinophilic esophagitis (EOE)

EoE is a global health condition, with an incidence of ~1 per 10,000 people in the US. Symptoms of EoE greatly impar quality of life. Individuals with EoE are predominantly young males with a high rate of atopic disease, and the diagnosis of EoE is made by endoscopy and biopsy findings of isolated eosinophils in the esophagus. Although the underlying cause remains unknown, EoE as other atopic d...
